You do not need to print the participation packet. An electronic version can be e-mailed to you by providing your e-mail at the end of the document after your signature.
Please do not use any leading zero at the beginning of your student’s school ID number.
You must have your athlete’s student ID number to continue with the online forms.
Online participation forms, along with the UIL Medical History and Pre-Participation Physical Exam MUST be completed and on file with the Athletic Trainers or Middle School coach PRIOR TO participation in any practice, scrimmage, performance, or contest before, during or after school.
Do not print online participation form, it will not be accepted.
